Contractor Change order form

A contractor’s change order form is a document that shows extra work done by the contractor that may not be indicated on the initial contract between the contractor and the client. It contains a description of the extra work and materials used, the time consumed, extension date and the new cost as a result of the extra work.

This form comes in handy as it shows that the extra work was done with the authorization of the client, hence it helps to avoid disputes or misunderstanding when claiming for compensation.
