Peer feedback forms are used to give information about people working or studying in the same team. This means that every member of the team will fill feedback forms on all the other members of the team to help provide different perspectives on matters and to identify any problem areas that need to be looked into.

Using the scale below, rate your group members and yourself on the task you just completed. The contents of this feedback form will not be made known to other group members.  Please be as honest and frank as you can.
